Each player on the FCA Lady Falcons Lacrosse Team is required to complete six forms. Each required form is listed at the bottom of this page as a link to PDF file. The forms can be downloaded and read with Adobe Acrobate Reader (download the free reader at www.adobe.com). Please click on the links at the bottom of this page to download the appropriate form(s).
Please note that completion of all forms and payment of all team fees must be provided prior to the start of practices on 15 February, 2011.
Parents/Players should give priority to completion and submission of the Registration Form and payment of the $50.00 registration deposit. The completed registration form will allow us to contact you via email. Additional forms can be submitted separately at a later date when the appropriate endorsements have been obtained.
